Your Children's Medical Information
We also offer medical forms and information. Specifically, it includes medical power of attorney forms you can use as needed as well as a summary of your child’s important medical information.
Notify Individuals Named to Care for Your Children
We take the next step and send letters to those you have named letting them know that they have been chosen to act in these important roles. We also send copies of the necessary documents so they will have it on hand in case of an emergency.
