Step 2. Provide us with copies of your statements, beneficiary designations, and any POLST or DNR documents. (Optional)

We recommend that you provide us with copies of your recent  statements and beneficiary designations so that we can review these documents with you during your appointment to confirm that they were properly transferred to your trust.

We also recommend that you provide us with any physician prepared medical records such as a POLST or DNR.

You have several options to submit your documents:

3. Mail or hand deliver your documents to Jill Gregory Law at 620 Newport Center Drive, Suite 1100, Newport Beach, CA 92660.
